Total Health Care (THC) Terms of Service and Consent

This applies to face-to-face and telepractice consultations

Our expectations of your conduct:

Respectful behaviour is expected at all times. We have a zero-tolerance policy for aggressive or abusive behaviour.

Privacy Protection:

Total Health Care follows privacy laws.

Informed Consent:

Informed consent means that you fully understand and agree to our terms of service.

Payment:

You need to pay for services at the time of the appointment. We no longer offer split billing. The person responsible for paying the bills in full is the individual static account holder.

A text message will be sent to you prior to your appointment time. Your ‘Y’ reply to this sms upon receipt is required for the appointment to be kept scheduled. If you do not reply ‘Y’ to the sms upon receipt, your appointment could be cancelled.

If appointments are cancelled by you at the last minute, you may have to pay a fee at the discretion of your practitioner. This fee is not claimable through Medicare. A 48 hour cancellation policy applies, unless you speak with reception (click here for contact details) with more than 48 hours’ notice prior to the scheduled appointment time. You also agree to indemnify Total Health Care and it’s employees, keep indemnified and agree that you will be fully liable for any claim, action, damage, outgoing or payment, etc. Total Health Care may suffer, incur, or be held liable for, by reason of non-payment for service(s) provided by the independent Total Health Care practitioners. This will include but is not limited to administration fees, debt collection agency fees, court costs, etc.

If there is an unpaid component of your previous bill, further appointments may be cancelled until the bill has been paid in full.

As our individual medical practitioners do not collaborate with each other on patient fees in order to comply with the Competition and Consumer Act 2010, individual practitioners will bill according to their individual fee structure.

You are personally responsible for any damage caused by you or the person(s) you have with you, to any Total Health Care property.

Our practitioners:

All Total Health Care practitioners utilise Total Health Care rooms and administrative services on a part-time basis.  They are not employees of Total Health Care.

Appointments:

No appointment will be made without your full completion of our patient details form.

All appointments are booked in advance and made at the discretion of the relevant practitioner.

All appointments with physicians or psychiatrists require that a valid referral is provided prior to the appointment date in order to receive any Medicare rebate.  Please note that Medicare rebates are only available for patients attending appointments whilst located in Australia.  If you attend an appointment whilst overseas eg. via Telehealth, you will be required to pay the full fee in advance of your scheduled appointment without the ability to claim a Medicare rebate.

All Total Health Care employees and individual practitioners want to ensure that you are provided with their best service.

Individual practitioners are able to provide scripts, forms and reports out of scheduled appointments. This will occur additional fees.

Payment will be required to be cleared in full prior to the script, forms or other reports being released or future appointments being made.

Changes to Terms and Ongoing Acceptance:

Total Health Care reserves the right to amend these Terms of Service and Consent from time to time. By making an appointment with, contacting, or otherwise using Total Health Care, you acknowledge and agree to be bound by the current version of these terms as published on our Total Health Care website. Continued use of Total Health Care services after any amendments have been published constitutes acceptance of the revised terms. You are responsible for reviewing these terms periodically to remain informed of any updates.
