HOW DO I BOOK AN APPOINTMENT?

WHAT IS TOTAL HEALTH CARE?

Total Health Care is a multidisciplinary health service working conjointly to provide medical, psychiatric, psychological, and dietetic treatments in one setting across the age spectrum.

Founded in 2008, the practice is led by Professor Michael Kohn and has a diverse group of experienced practitioners from a wide range of disciplines.

DO I NEED A REFERRAL TO SEE A PRACTITIONER AT THIS CLINIC?

Yes, if you would like to submit a claim through Medicare in order to receive a rebate, a referral is required.

Referrals from your GP are valid for 12 months, whilst referrals from Specialists are ONLY valid for 3 months.

If you do not have a valid referral for your visit, you will not be eligible to claim any Medicare rebate for your visit. Therefore you will be required to pay the full private fee without any Medicare rebate.

WHAT IS THE CANCELLATION POLICY?

A 48-hour cancellation policy applies unless you contact reception (click here for contact details) with more than 48 hours notice during business hours prior to the scheduled appointment time.

HOW CAN I TRANSFER MY MEDICAL DOCUMENTS?

If you have been treated at another clinic and would like to transfer your records to us, please download and complete the Consent Release Form to THC.

If you have been treated at our clinic and would like your records to be transferred to another clinic please download and complete the Consent Release Form to another clinic from THC.

IS IT POSSIBLE TO HAVE A TELEPRACTICE APPOINTMENT INSTEAD OF A FACE-TO-FACE?

Yes, at Total Health Care we offer telepractice appointments as well as face-to-face appointments.

WHAT IS A MENTAL HEALTH CARE PLAN (MHCP) FOR?

A Mental Health Care Plan (MHCP) is provided by your GP. You will be able to claim a portion of the consultation fee as a Medicare rebate. Patient MHCP referrals are typically valid for 1 year from the start date of the referral.

Patients are eligible for up to 10 sessions per calendar year. Initially, patients are able to access 6 of the 10 sessions under their plan. A review by your GP is required for the subsequent further 4 sessions if required within the calendar year.

WHAT IS THE DIFFERENCE BETWEEN A EPC/CDM & EDP?

Enhanced Primary Care (EPC), also known as Chronic Disease Management (CDM), in which a GP services a plan to enable the coordination of a patients multidisciplinary, team-based health care. A patient suffering with a chronic or terminal medical condition who would require care from their GP and at least two other health or care providers. You will be able to claim a portion of the consultation fee as a Medicare rebate.

Eating Disorder Plan (EDP), is provided by your GP to delivery psychological or dietetic treatment for a clinical diagnosis of an eating disorders. This plan would allow a claim to be made for a portion of the consultation fee as a Medicare rebate. 40 psychological consultations in a 12 month period and dietetic consultations up to a total of 20 in a 12 month period.

DO YOU SEND OUT APPOINTMENT REMINDERS?

A text message will be sent to you prior to your appointment time.  If you do not reply to the sms or cannot be contacted within 48 hours prior to your appointment time, you appointment could be cancelled.
